If your plan is not listed, please call the office at 208-898-0304 to determine if we are providers for your medical insurance or vision plan. Our Meridian eye doctors also provide Medicare eye exams and eye care.
Each patient is responsible for verification of their coverage. We will be happy to assist you with on a limited basis with your authorization process. If your verification and authorization is not pre-approved before your visit you will be responsible for full payment of professional fees and materials on the day of your visit. We are unable to make any exceptions to this policy.
